Developing Compassionate Communities in North West Melbourne.
Working with local government areas; Hume City, Yarra City and Melbourne City.
At the invitation of the North West Melbourne PHN, GroundSwell tailored a Compassionate Communities model to mobilise community members to be more active in talking about and planning for end of life.
This project sees key stakeholders working together and the project being largely informed by the lived experience of community members. A key partnership with Preparing the Way End of Life Doulas has formed a strong foundation of the work with end of life doulas actively working in the context of their own local areas, bringing a mix of community development experience and end of life sector expertise.
COVID-19 has forced this initiative to pivot away from the planned face to face community events and will now be focusing on digital ways to connect and transfer information and support.
